Two active Pokémon at once

After one of my Pokémon was knocked out the game automatically promoted my mew ex just before I promoted my Moltres V and I ended with two active Pokémon. I could not attack or retreat at all and when I tried to switch with escape rope the new Pokémon just went on top of the Mew and I still couldn't play.

This sounds incredibly odd. A battle log might be able to help us.

Unfortunately I forgot to save the battle log but when I checked it after the game it does show Mew ex as still being on my bench.
