IVs can only be increased by Hyper Training, which costs a bottle cap / gold bottle cap. The people you need to talk to to Hyper Train are in Montenevera and the Fist of Justice Dojo in SV and Z-A respectively. They can't be boosted by a specific amount or decreased or altered in any other specific way, only maxed out.
You unlock the ability to check them in SV by talking to the Pokémon Center nurse after beating the game, and in Z-A it's instantly unlocked after the credits. You can't use them on the full summary screen, just the preview version that appears when scrolling through boxes, meaning it can't be seen for your party Pokémon outside of boxes. I don't remember which button it used, but you'll know when it puts dialogue under each stat's name.
I found a list of judge dialogue and corresponding IVs, so here it is:
IV of 0: "No good"
IV range of 1-15: "Decent"
IV range of 16-25: "Pretty good"
IV range 26-29: "Very good"
IV of 30: "Fantastic"
IV of 31: "Best"
The exact number of IVs doesn't matter unless you have a rare strategy that involves not maxing out a stat. Usually it's best to just use bottle caps on all that aren't already 31.
EVs, however, can be seen on the full status screen but not the mini version in boxes. Look at the stats graph and press L in SV and Z-A. Also, one thing I forgot to mention, Power items (the Power Anklet, Power Bracer, Power Belt, Power Lens, Power Band, Power Weight) usually increase EV gained for a specific stat (with the trade-off of slowing the Pokémon until you remove them).

And it says "Hyper Trained!" if you Hyper Trained them, I think.

I Hyper Train all my Pokémon
If they already have prefect stats in certain sections, then the trainer won't Hyper Train that stat
My Charizard in Z-A has three Best stats so the Hyper Trainer couldn't do anything else with them

No, vitamins affect EVs (which have to be gained and can be increased and decreased). IVs (determined when you obtain a Pokémon, can only be changed into the maximum through Hyper Training and can't be decreased or increased). The reason your Charizard's stats are like that may be related to events or gift Pokémon (or really, really good luck). I don't know much about how they function in Z-A, so I don't know which things have guaranteed perfect IVs.
I do remember how they worked in SV though, Tera raids had an amount of perfect IVs for the Tera Pokémon based on the amount of stars. 7 Stars get perfect IVs in all 6 stats, 6 get perfect IVs in 5 random stats, 5 stars are the same for 4 random stats, 4 stars get perfect IVs in 3 random stats, 3 stars in 2, 2 and 1 stars get perfect IVs in one random stat. Events vary and I remember legendaries having a few guaranteed perfect IVs in random stats, but I don't remember the exact amount of them.
