How can Pokemon have 2 items?

I was playing ranked battles and I saw a mega Salamence with a rocky helmet. No screen shot but, it was weird. I could not see the players name but, my friend says it was Japanese. It could be another Pokemons rocky helmet activated but all the other pokemon were mega evolved.

    If you're talking about Champions no... each Pokemon can only hold one item.
