It never appeared in any game or anime, but Mewthree did its only appearance to date in Pokemon Pocket Monsters manga.

So perhaps Mewthree is around somewhere, maybe one day it wil surprise us.

I found it while looking for images of Mewthree. It was promotional art for the Mewtwo movie remake, made by the artist who made the Pocket Monsters manga.
