Was that Pokémon switched to the Bench, then was switched back to the Active Spot again? Doing so will remove that attack's effect on that Pokémon.

You can't apply more than 3 (of 5) Special Conditions to your opponent's Active Pokémon at a time. It can be Burned and Poisoned, but for the rest (Asleep, Confused, and Paralyzed), you can only apply 1 more of them.
